Programme: Incident Reporting & Root Cause Analysis Workshop

Synopsis

A training session concentrating on Incident Reporting and Root Cause Analysis (RCA) was conducted to educate department representatives about RCA procedures and the importance of reporting clinical incidents. Professor Dr. Mohd Idzwan Zakaria, a highly experienced Consultant Emergency Physician in RCA and clinical inquiry, was invited to facilitate this session.

The workshop aimed to achieve the following objectives:

• Furnishing department representatives with an understanding of incident reporting methodologies and the protocols involved in conducting Root Cause Analysis (RCA).
• Explaining the tools and techniques necessary for analyzing failures within work processes, particularly in relation to incident occurrences.
• Promoting a culture of continuous improvement by urging each department to proactively implement corrective actions and consistently enhance their services.
